Summary

Minimum starting salary: $15.0000 hourly

 

Under direct supervision, the Community Health Worker (CHW) I is responsible for helping community members and their families navigate and access community services, community resources, and adopt healthy behaviors. CHW is a core member of the Community and Clinical Health Bridge program aimed to improve obesity and related chronic disease prevention and care.

 

Primary Duties

 

Essential functions, as defined under the Americans with Disabilities Act, may include any of the following representative duties, knowledge, and skills. Factors such as regular attendance at the job are not routinely listed in job descriptions but are an essential function. Essential duties and responsibilities may include, but are not limited to, the following:

 

• Provides culturally appropriate health education, information, and outreach in

• community-based settings (e.g., nutrition, physical activity, weight management and disease self-management education).

• Establishes and maintains trusting relationships with community members and their families.

• Utilizes motivational interviewing techniques with community members and referred clients to identify and set individual and/or family health goals.

• Ensures that clients are referred to appropriate health services, resources and/or programs based on individual/familial need and set health goals

• Helps individuals, families, and the overall community understand their health condition(s) and develop strategies to improve their health and well-being.

• Manages assigned caseload of community members by maintaining regular communication and follow-up protocols

• Documents all client encounters and contacts made on behalf of clients; completes and submits monthly reports; maintains comprehensive client files, which include client notes, release of information, assessments, and other documents.

• Attends regular staff meetings, trainings, and community outreach events as required (including those that are outside of normal business hours).

• Supports the relationship between the City of Waco and the general public by demonstrating courteous and cooperative behavior when interacting with citizens, visitors and City staff; maintains confidentiality of work-related issues and City information.

• May be required to respond after hours, including holidays and weekends, in the event of a departmental or City-wide emergency.

• Performs other related and assigned duties as required.

• Driving is essential.

 

Qualifications

 

Required:

 

• High School Diploma or GED equivalent.

• Volunteering experience, working with community organizations or non-profits is required.

• Must possess a valid Texas Driver’s License.

• Must be certified by the Texas Department of State Health Services Community Health Worker (CHW) within 6 months of hire.

 

Preferred:

 

• Associate degree in a related field.

• Bilingual Skills.

 

Physical Demands

 

Sedentary work:

 

• Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Sedentary work involves sitting most of the time.
